Volunteer Requirements

Would you like to volunteer?

Blackhawk is thankful for all community members willing to volunteer their time as a district volunteer.  Here are our volunteer procedures: 
  • All district volunteers must provide a copy of the volunteer affidavit and clearance forms yearly to one of the district buildings. Clearances only need to be sent to one building. We request that clearances are submitted by September 1st.  
  • Once clearances are received and checked, the volunteer’s name will be added to the district volunteer database.  
  • Volunteers will receive an email confirming that they are included in the district’s volunteer database.  
  • All clearances will be disposed of at the end of the school year.  
 

Here is what you need:

1. Volunteer Clearance Submittal Form-- no cost, every volunteer needs to submit a new form each year.

 
2. Volunteer Confidentiality Agreement-- no cost, must present ID with submittal of agreement.  Required each year.
 

3. Pennsylvania Criminal Record checks (SP4-164) -- no cost, every volunteer needs to submit a new form every year.

 

4. Pennsylvania Child Abuse History Clearance (CY113) -- no cost for 1st time, and volunteers need to renew every 5 years. (For renewals before 57 months there is an $8 fee).


5. Submit one of the following:

A. If you have been a Pennsylvania resident for the entire previous 10 years you may submit the Unpaid Volunteer Affidavit.
 

B. If you have not been a Pennsylvania resident for the entire previous 10 years you must have a Federal Bureau of Investigations Criminal Background Check - $27.00 (use link under the Department of Education). Login using Service Code: 1KG6XN

Where do I take clearances?

You can bring your clearance documents to any building office or you can email the documents to the following secretaries:
